Investigation Summary
Employee #1 was on a crew installing metal roofing over insulation. He was at the leading edge of the building when he bent over to straighten out the insulation. Coworkers later stated that a gust of wind dislodged a sheet of metal. Employee #1 was attempting to catch the metal when he stepped through the insulation and fell 30 ft to the concrete floor. He suffered severe head injuries and was pronounced dead at the scene.
