Employee Collapsed While At Work And Dies

At approximately 8:30 p.m. on October 14, 1998, Employee #1 collapsed while at work from unknown causes. Prior to collapse, Employee #1 had been installing cable wire from an elevated work platform attached to a forklift. No witnesses observed Employee #1 hit his head or act strangely prior to his collapse. Employee #1 was transported the hospital and admitted. Employee #1 died in the hospital two days later.
