Employee killed due to fall from truck

At about 11:30 a.m. on December 30, 1998, a truck driver was in the process of unloading paper slush at a Dairy farm in Rikerall, Oregon. The employee had positioned himself on the rear step area of the tractor, between the chassis, to check on the load being unloaded via a walking floor conveyor. He fell backwards and landed on the ground just in front of the tractor dual wheels. A few minutes later, he was found dead by other employees.
