Toe of Employee Amputated by Coupling

Employee #1 was clearing a jam on the unscrambler. One rotating shaft in the area where he was working did not get shut off. He was straddling the conveyor and placed his foot by the rotating shaft and coupling. The coupling had a protrusion that rotated around and amputated his small toe. Employee #1 was hospitalized for his injury.
