Employee killed when head caught in gun seal machine

Employee #1 was cleaning broken glass from underneath the Gun Seal Machine Number 2 while it was still in operation. The rotating parts of the machine were not guarded. Apparently his head came into contact with the open area under the machine as the gun seal indexing table was rotating around 20 tube stations. Employee #1's head was struck by and caught between a vertical member and an arm of the machine. Employee #1 sustained lacerations on the right and left side of his head. His calvarium showed bilateral depressed skull fractures in which a disk shaped segment had been traumatically mobilized and pressed against the brain.
