Employee Killed In Fall From Ladder

Employee #1 was standing on the third step down from the top of a 15-ft stepladder. Employee #1 was reaching up into the 18-ft high ceiling of an auditorium in order to place a cloth strap and rope around a girder. The cloth strap and rope weighed approximately two pounds. A coworker was standing on the floor, holding the ladder. The coworker looked up, saw that Employee #1 was off the ladder, hanging onto the girder. The coworker climbed the ladder, placed Employee #1's feet back on the ladder and told Employee #1 that his feet were back on the ladder. Then Employee #1 lurched up, not saying anything and fell to the carpeted floor, landing on his head and shoulders. During the entire time Employee #1 did not call out or indicate he was in trouble. Employee #1 was transported to the hospital where he was pronounced dead. The autopsy report from the medical examiner indicated Employee #1 died from hypertensive heart disease.
