Investigation Summary

Investigation Nr: 170831739
Event: 05/15/1998
Employee's finger amputated in cookie portioning machine

Employee #1 was operating a cookie portioning machine that used an 18 in. long by 4 in. wide blade to cut wax paper into pre-determined lengths. He was reaching through a hole in the guard to move cookie dough when the blade amputated his left index finger at the first joint. Employee #1 was flown to Union Memorial Hospital, where doctors reattached his finger, although the prognosis was not good. The machine's plastic-type guard with interlocks was missing a piece on the left side.
