ELECTRIC SHOCK - CONTACT WITH ENERGIZED PARTS THRU WRENCH

An employee was replacing a thermocouple at the back of a vacuum oven. His employer had no lockout/tagout program, and the 480/240-volt heater remained energized while he was working. The adjustable wrench the employee was holding in his right hand contacted the heater lead, and the watch on his left wrist was apparently touching the machine frame. The employee received an electric shock and lost consciousness. A coworker heard a scream and called for emergency medical services. The responding emergency medical services team administered cardio-pulmonary resuscitation and defibrillation. The team transported the injured employee to a hospital, where he remained unconscious for 4 days. The employee subsequently recovered and returned to work. However, he retained no memory of the accident.
