Investigation Summary
An engineer and a junior engineer working for a shopping mall went to the roof of the mall to investigate a problem with the air handler unit. They opened the disconnect box and discovered that the three-phase, 480-volt disconnect needed to be replaced. The engineer called a supply store, but an exact replacement was not in stock. He remembered that there were two spares in the main panelboard area in the electrical room. He decided to pull one out and use it for the air handler. The engineer removed cover from the main panelboard without deenergizing it. He asked for a flashlight, and his assistant gave it to him and turned around to read some gauges. The engineer, who had a screwdriver in one hand, was looking in the back of the panelboard with the flashlight when an electrical fault occurred. The ensuing electric arc burned the engineer and threw him 4.5 to 6.0 meters away. The shaft on the screwdriver showed signs of melting after the accident. The junior engineer put out the fire with a fire extinguisher and helped the injured employee off the floor and to the office. The engineer was hospitalized for his injuries.
