Investigation Summary
On January 30, 1999, Employee #1 was operating a traverse forklift tractor, model TL6035, serial #60653-CCC, at a construction site in Ponchatoula, LA. The rear tire on the driver's side was low, and he was moving the lift from near building #14 to an area where a coworker was waiting to change the tire. The ground was muddy from recent rains, and as the forklift moved onto a grade, it began to slide. Employee #1 raised the forks to clear a tree, but as the lift began to slip and become increasingly unstable, he tried to jump from the cab and get clear. He was 15 to 20 ft away when he was struck across the back by a section of the forklift that connected the carriage to the extensible boom. Employee #1 sustained multiple injuries and was pronounced dead at the scene. The company had no training program for lift operators and had no way to determine their competency. Forklift operation was Employee #1's primary job at this work site, and it was believed that he had operated such machines at other locations.
