CONSTRUCTION EMPLOYEE KILLED WHEN STRUCK IN HEAD BY COIL

HEATING COILS WERE BEING REMOVED FROM A DRY KILN FOR RETROFITTING. A HYSTER FORKLIFT WHICH WAS IN NEED OF SERVICING WAS BEING USED TO LIFT THE COILS. A SECTION OF COIL, 150 INCHES LONG AND 67 INCHES WIDE, WEIGHING 1500 POUNDS, SUPPORTED BY A CHAIN DRAPED OVER THE LIFT'S FORKS, WAS LIFTED TO A HEIGHT OF 2 FEET. THE FORKLIFT SUDDENLY SLIPPED INTO REVERSE, CAUSING IT TO JERK OUT FROM UNDER THE CHAINS AND ALLOWING THE LOAD TO DROP. THE COIL STRUCK EMPLOYEE #1 ABOVE THE KNEES, KNOCKING HIM TO HIS KNEES. HE THEN WHIPLASHED BACKWARDS, STRIKING HIS HEAD AND SUFFERING A FATAL CONCUSSION.
