Investigation Summary
Employee #1 was on the roof of a building at the job site when he suffered a massive subarachnoid hemorrhage, secondary to a ruptured berry aneurysm. He fell approximately 6 ft onto some scrap asphalt roofing shingles in the dump body of a dump truck. Employee #1 was killed, although the fall did not contribute to his death. He did not work for the company but was visiting his cousin, the company owner, at the time of the accident.
