Employee killed in fall from ladder

Employee #1 was on a portable A-frame ladder in an elevator shaft recessed approximately 5 ft from grade. He was using a gasoline-powered concrete cutter when he fell and sustained a head injury. Employee #1 was transported to the hospital, where he died five days later.
