Employee killed in fall from fixed ladder on rail car

Employee #1 was preparing to unload automobiles from the upper level of a three-tiered railroad car. He used a fixed ladder to access the top level and was reaching out to grab a safety rail, but missed it. Employee #1 fell backward onto the pavement, sustaining fatal head injuries.
