Employee killed in fall from scaffold

On April 13, 1999, Employee #1, working for a subcontractor, was inside an office/warehouse building under construction. He was climbing the outside of a 19 ft scaffold to weld the ceiling beams. At the top of the scaffold, he was stepping onto the platform and reached for a ceiling beam for support. He had difficulty holding on and fell onto the concrete floor, landing on his back, head, and neck. Employee #1 was killed. The lack of guardrails on the scaffold contributed to Employee #1's fall.
