Investigation Summary
At approximately 10:50 a.m. on June 5, 1997, Employee #1 backed up a semi-tractor with an attached end-dump trailer to an aggregate pile to deliver a load of rock. The tractor and trailer jackknifed to nearly a right angle. As Employee #1 was dumping his load, the trailer twisted and tipped onto the cab, pinning him against the steering wheel and dashboard. Emergency 911 was called and the local fire department, police, and ambulance all responded. Employee #1 was pronounced dead at the scene; he was transported to the Mercy Hospital morgue.
