Investigation Summary
On June 10, 1997, Employees #1 through #3 were working from a tubular welded-frame scaffold to hang drywall in a foyer of a two-story house. The scaffold platform board broke, and they fell approximately 10 to 15 ft onto a concrete floor. All three employees sustained fractures to the left leg or ankle, and were hospitalized. The incident was not reported by the employer or anyone else until 24 days after it occurred.
