Employee dies of asphyxia when caught by forklift mast

Employee #1 was using a Caterpillar forklift, serial #82F6220071, to load pallets of processed turkey meat into a blast cooler. Apparently he dismounted, leaving one foot on the seat, and was leaning forward between the mast and overhead protection frame when he inadvertently engaged the lift and/or the tilt control as he stood up. His neck became caught between the forklift mast and the overhead protection bar. He died of asphyxia due to suffocation.
