Employee exposed to ammonia when pipe flange leaks

At approximately 7:30 a.m. on June 16, 1998, Employee #1 was performing his duties when between 1 and 2 lb of ammonia gas leaked from a pipe flange. No injuries or illnesses were reported as a result of the accident. The bolts had worked loose due to temperature changes; they were tightened and this task was added to routine maintenance procedures.
