Investigation Summary
Employee #1 was in a 45 ft long trailer, using a Toyota forklift, model 6FGCU20, to unload pallets of freight. He had driven in, gotten the forks under a pallet of cargo, lifted it, and then reached through to put a sticker on the boxes or to read the block. As he did this, he apparently pushed the lift lever forward, lowering the load and pinning him at shoulder level in the 19 in. wide opening between the mast and the frame. Employee #1 was crushed and killed. The levers--one to tilt the mast, the other to lift the forks--were both on the right-hand side of the steering wheel.
