Investigation Summary
Employee #1 and a coworker were pressure washing an arch-type roof prior to painting it. The coworker was using the pressure washer, and Employee #1 was on a lower roof assisting with the pressure and hoses. Employee #1, who was not wearing any form of face or fall protection, went up on the arched higher roof and fell through a skylight. He was killed.
