Investigation Summary
Employee #1, a loader operator at a concrete plant, noticed that some of the drive belts had come off a conveyor transmission. He went to get some tools and bolts, and then told the plant manager not to load any trucks because he was going to repair the problem. As Employee #1 was putting the belts back on the conveyor pulleys, the 25 hp motor activated the three 1/2 in. pulleys, catching his hand. The motor stopped almost immediately, and Employee #1 was able to free his hand. He suffered a fractured thumb, a lacerated little finger, and the nails were torn off his third and fourth fingers. The motor and pulleys operated at 1,760 rpm.
