Employee injured in fall descending fixed ladder

Employee #1 was inside a water tower under construction, helping to weld the sides in place. He was descending a fixed ladder inside the center pole when he lost his footing and fell 64 ft to the ground. Employee #1 suffered multiple fractures, abrasions, lacerations, and contusions. He was wearing a full body harness and the fixed ladder was had a ladder safety device, but he was not hooked up to it.
