Investigation Summary
Employees #1 and #2 were in a 5 to 7 ft deep by 24 ft long trench, cleaning the ends of a 12 in. diameter by 20 ft long water line in preparation for connecting it to another pipe. The bank of the trench sheared off, pushing the pipe against the employees. They were covered to their waists and sustained broken bones and bruises.
