Investigation Summary

At approximately 9:45 a.m. on June 13, 1997, Employee #1 was repositioning crossouts on a flatbed trailer in the loading area of a shipping department. A coworker was driving a forklift on the opposite side, preparing to load four bundles of lumber onto the trailer bed. The coworker moved closer to the trailer and had stopped to lower the four-pack when the top outer bundle fell, striking Employee #1. He sustained multiple head and chest trauma injuries, and was pronounced dead at the scene.
