Investigation Summary
Employee #1 collected cardboard materials using a 1994 Freightliner LD30 recycle truck. The lid of the truck closed, pinning his head and neck area between the bin lid and the truck body. Employee #1 was found facing the truck, hanging by his neck and hands, feet hanging off the horizontal truck bin framing member. Employee #1 was killed.
