Employee fatally injured from slip and fall

On July 22, 2001, Employee #1 was working in a tollbooth when he slipped. He fell hitting his head on a surface that caused a laceration to the head. The size of the laceration was 3 in. to 4 in. long. He died from cardiovascular shock due to the large amount of blood loss.
