On August 20, 1999, Employee #1 was in a parking lot with his tractor and trailer. Employee #1 had stopped his tractor on a slight incline and had departed the vehicle while the engine was running, the emergency brake was not set, and the transmission was in the neutral position. Employee #1 was found crushed between the tractor cab door and the trailer.
