Investigation Summary
Employee #1, a part-time employee of several months, was operating a fork lift moving a pallet to an area outside the building. He was operating at a high rate of speed and tipped the truck over when he attempted to make a turn. The forklift overturned onto Employee #1, crushing and killing him. Employee #1 was not wearing a seat belt, although the truck was provided with one.
