Employee killed when crushed by machine

On or about August 30, 1999, an employee had climbed inside the EVG wire mesh machine to replace a limit switch. After replacing the switch, and while still inside the wire-feed area of the machine, the employee signaled the foreman, who in turn signaled the operator to start the machine. When this was done, the line-feed mechanism moved forward crushing and killing the employee. The activity was not performed under the protection of lockout-tagout.
