Employee Loses Fingers In A Malfunctioning Mechanical Press

Employee #1 lost part of her index and middle fingers on her right hand while she was working with a malfunctioning mechanical press. She is primarily responsible for operating the press controls. She placed her right hand in the 70-ton Minster OBI Part Revolution Press. She was attempting to realign the part that the maintenance person had placed in the die area. This part measured 3.875 feet. The press had been inspected by the operator. However, she had complained that when activated by the palm buttons the press would sometimes fail to function. A maintenance person came to fix the press. Employee #1 was standing behind the maintenance person and reached around him as he activated the press. As a result, she partially lost the index and middle fingers on her right hand. Weekly maintenance records were not maintained, and Employee #1 was not properly trained. She was hospitalized for treatments of the partial amputations.
