Investigation Summary

Investigation Nr: 202085429
Event: 03/04/1998
Employee killed as wall and cement truck fall on him

Employee #1, who was the owner of William Celek Construction, was directing a concrete truck into position to pour cement into the foundation of a house that was being built. He was standing on the top of several metal forms that enclosed one side of the basement. Employee #2, his son, was working on the floor below the wall. As the cement truck moved into position, the ground under the truck gave way. Although the driver attempted to back-up, the truck continued to slide sideways toward the edge of the hole. Employee #1 jumped into the basement and pushed Employee #2 to safety just as the wall partition and the truck fell into the hole on top of Employee #1. He was killed instantly.
