Investigation Summary
Employees were experiencing trouble aligning trusses that were being manufactured. The plates that join the wood together on the trusses had interlocked with each other binding several of the trusses together. The employees tried to free the trusses on the truss table but were unable to do so. There were 21 trusses in the load and each truss weighed approximately 300 lbs. Employees decided to remove the trusses from the table with the forklift. The load was then placed on the ground next to a telephone pole standing upright. Employee #1 walked over to the employee driving the fork truck to explain how to fix the problem and the trusses fell, fatally pinning him to the ground.
