Investigation Summary
Employee #1 was performing preventative maintenance on the Banbury mixer when his head, arm and shoulder entered the opening of the feed door of the mixer. The hydraulic door was actuated, and Employee #1 was pinned between the door and the mixer. The machine was not locked out at the time of the accident.
