Investigation Summary
At approximately 7:30 a.m. on October 24, 2002, an employee riding an All Terrain Vehicle (ATV) to check on the fill ponds and control the water flow from the ponds to the river. The employee went to check the ponds and water flow and for unknown reasons the employee rode the ATV into the pond, which contained approximately 10 ft of water where he drowned.
