Investigation Summary
On January 22, 2004, Employee #1, an employee of the U.S. Forest Service, was removing material from the back of truck onto a loading dock when a section of ice and snow slid from the roof and fell approximately 6 feet, striking and crushing him. The section of ice, which measured 4 feet by 4 feet, slid from a metal decked with a 4-in-12 pitch. Employee #1 died.
