Investigation Summary

Investigation Nr: 202033536
Event: 12/10/1999
Employee killed in explosion of van by pyrotechnic material

Employee #1 and Employee #2 were burning waste pyrotechnic material in a gravel pit located within a fenced fairground facility. The method used involved spreading the material in the pit area and then igniting the burn with an electronic match device. A van containing hundreds of pounds of pyrotechnic waste material was driven into the pit during the set-up process. The two workers would wait a short period and check for hot spots before starting to spread out more material. While beginning to spread material for the third burn, a hot spot apparently caused a spark to fly into the van and ignite the explosive material in the van. Employee #1 was killed in the blast, while Employee #2 sustained minor burns. The cause of death of Employee #1 was determined to be massive head trauma.
