Employee killed when head struck by falling forklift fork

Employee #1 worked for a company that leased space in a marine terminal. He was responsible for picking up trash in the garage, which was primarily used for truck maintenance. He was emptying trash from an ore bucket attached to the blade of a 2-ton Clark forklift into a 20 ft dumpster. Standing with one foot on the right-hand-side running board and the other foot placed against the side of a van that was parked alongside, Employee #1 was trying to trip the latch on the ore bucket to allow the trash to be released into the dumpster. The forks were raised approximately 8 ft above the ground. The forklift blade that was not being used to lift the ore bucket fell off the carriage and struck Employee #1 on the head. He was killed.
