Investigation Summary
At 1:15 p.m. on August 27, 1996, Employee #1 was completing the installation of a gas fireplace assembly for a new house under construction. He was using a 25 to 30 ft tall ladder to install flue piping components to the top of the chimney. The ladder was still in place when Employee #1 fell to the ground. He died of blunt trauma injures to the head.
