Investigation Summary
At approximately 11:50 a.m. on February 27, 1999, Employee #1 was working in an excavation, approximately 30 ft long, 3 ft to 5 ft wide, and 10 ft to 12 ft deep, when a section of the north wall collapsed. A bystander was standing at the west end of the excavation when the collapse occurred and summoned help. Employee #1 was killed.
