Employee killed when crushed by powered industrial truck

Employee #1 was standing on a powered industrial truck between the mast and the steering or control area. Employee #1 was trying to place a 5-foot by 5-foot vent hood onto the tines of the truck. An operator was seated in the truck and was raising and lowering the tines. As Employee #1 stepped back, his foot made contact with the controls and he was crushed by the mast and killed.
