Investigation Summary

At about 10:00 a.m. on February 17, 1999, Employee #1 was working in a hole dug by a backhoe for the placement of a reinforcement fence post. Another employee, returning from the shop area to the site, was driving a pickup and approached the site from above the hole. Although the vehicle speed was slow, the slick conditions of mud and wet snow caused the pickup to slide onto the hole, striking and pinning Employee #1. He sustained a fractured hip and was hospitalized.
