Employee Is Injured in Fall from Roof

On October 17, 2005, Employee #1, a supervisor with Applied Coatings & Linings, Inc. (a privately owned corporation that engages in the blasting and painting of various metal parts), was on an extension ladder applying plastic to a 16-ft-high roof in wet conditions. He slipped, slid off the roof, and struck a protruding pipe while falling to the dirt ground. Employee #1 was hospitalized at County USC Medical Center for treatment of a fractured back, and surgery for a fractured right leg and foot. Earlier, Employee #1's supervisor had twice denied Employee #1's requests to place plastic on the leaking roof.
