Investigation Summary
On June 4, 2005, Employee #1, a temporary employee with Staffmark, was cleaning a pair of rolls in a caramel extruder in the Nellson Nutraceutical's production area. He was standing on a stepladder that was unstable due to uneven feet. With the hopper removed and the extruder turned off, he scraped caramel off a portion of the extruder rolls. He then turned on the extruder to rotate the rolls to continue the cleaning process. While the rolls were rotating, the ladder rocked. Employee #1 leaned forward to catch his balance. The spatula he was holding became caught at the in- running side of the rolls. His right hand was then crushed by revolving rolls. Employee #1 was hospitalized for an amputation of the hand.
