Investigation Summary
At approximately 11:45 a.m. on April 22, 2005, Employee #1 and two coworkers, truck drivers, were loading a truck with the merchandise in a warehouse. One coworker was in the trailer and another coworker was operating a Crown manufactured forklift with the Model Number of 4000 Series. Goods were placed with a forklift on a pallet, which was on the back of the trailer. The pallet was not placed straight. While second coworker was trying to straighten the pallet, Employee #1 was standing on the dock at the rear of the trailer. To straighten the pallet, the second coworker tried to push the pallet slightly with the forks of the forklift. He pushed the pallet as he was turning the steering wheel of the forklift. The forklift swung when the second coworker accelerated it and struck Employee #1. Employee #1 fractured his pelvic bones and sustained internal organ injuries. He was hospitalized for 6 days.
