Employee Fractures Forearm in Fall from Platform on Forklift

On March 23, 2005, an employee who was inventory clerk was standing on a platform placed on the forks of a forklift, resting at floor level. The forklift was a CAT lift truck Number 972023. The platform had a chain across one side. The employee tripped over the lower chain on the platform. He fell onto the floor, landing on his wrist, fracturing two bones in his forearm. At the time of the accident, the employee was taking inventory of a shipment of toys at a toys and leisure goods distribution warehouse, where he worked as a temporary employee through a staffing agency. He was not hospitalized.
