Investigation Summary
On April 13, 2005, an employee was working for Indalex Aluminum Solutions, a company that extruded aluminum. Press 11 had problems maintaining the proper contour for the extruded aluminum. To fix this problem, the employee made carbon set-ups and placed them at different locations to restrain the metal product from moving. While the employee's right hand was holding on to the components of one of the carbon set-ups in order to keep it steady, one or more components of the set-up made contact with the moving product that was being extruded by the press. The right hand of the employee struck the water quench box. When his hand struck the quench box, the employee's right middle finger was pinched against the box, and amputated.
