Event: 04/05/2003
Employee Injured In Fall From Conveyor

At approximately 6:03 p.m. on April 5, 2005, Employee #1 was part of a cleaning crew that cleaned some of the machines. Employee #1 was cleaning conveyor belt Number 7 during a lunch break. Conveyor belt Number 7 delivered recycled materials from the plant and dropped them onto the truck. While Employee #1 was removing materials entrapped on the conveyor pulley, a coworker started the machines. This caused Employee #1 to fall approximately 14 ft from the conveyor belt to the floor. The employer representatives stated that conveyor belt Number 7 was not to be cleaned during lunch, but Employee #1 was on conveyor belt Number 7 to clean it during lunch. The machine was started without knowing Employee #1 was on the conveyor belt. Employee #1 fractured his left wrist and backbone and was hospitalized.
