Employee Fractures Leg and Ankle When Caught in Conveyor

At approximately 5:00 a.m. on January 29, 2005, a supervisor was standing on the elevated platform supervising Employee #1 and several coworkers as they attempting to join the two sections of a conveyor belt on the trash conveyor Number 3b. The employer did not lockout or disengage the power source to prevent inadvertent movement of the conveyor while being repaired. Employee #1 had started the conveyor motor and was pushing down on the conveyor belt at the roller so the belt would make contact with, and be moved by the roller. One coworker was also standing on the conveyor belt a few feet away using his feet to push the belt towards the open or south end of the conveyor. Other coworkers were standing on the frame of the conveyor and using their hands to pull the conveyor belt. It was necessary to move the conveyor belt to the south end of the conveyor in order to install the pin into the metal lacings on each end of the belt. During this operation, while Employee #1 was standing on the belt and pushing down on the belt to make contact with the roller, some slack developed and that slack was pulled into the roller. Employee #1's left leg became entangled in the slack portion of the conveyor belt. A coworker hit the emergency stop button and helped get Employee #1"s leg free. Employee #1 sustained multiple fractures of the left leg and a crushed left heel and was hospitalized for eight days.
