Employee is injured when caught in machine

On June 5, 2004, Employee #1 was working for a company that manufactures cardboard packaging. A series of conveyors palletizes boxes and ties them. The conveyor system is equipped by a pusher (to move the pallets) that is activated by photosensitive eyes. When the top layer of the palletized boxes fell, he climbed up onto the conveyor to remedy the problem. The pusher was then activated and his feet became caught. Employee #1's right fibula and both ankles were fractured.
